Who We Are

WoundScore is an AppleSoft Medical Solutions product powered by Zentric AI. WoundScore.com is the marketing and licensing website for the WoundScore wound assessment platform. References to "we," "us," or "our" in this Policy refer to the WoundScore partnership between AppleSoft Medical Solutions and Zentric AI as it relates to this website.

This Policy covers WoundScore.com only — the website you are currently visiting, which handles account registration, subscription management, and product information.

Important: We Do Not Collect Patient Data

WoundScore.com does not collect, store, or process any patient health information, medical records, wound images, or Protected Health Information (PHI) of any kind.

The clinical application that clinicians use to assess wounds and generate OASIS-D1 documentation is a separate product operated by AppleSoft Medical Solutions under appropriate Business Associate Agreements (BAAs). That clinical environment handles PHI under its own HIPAA-compliant infrastructure.

If you are a patient and you have questions about how your health information is handled, contact your healthcare provider or AppleSoft Medical Solutions directly.

What Information We Collect

We collect only the information needed to create and manage your WoundScore account and subscription.

Information you provide directly:

  • Name
  • Work email address
  • Professional role or title (e.g., nurse practitioner, physician assistant, agency administrator)
  • Organization name
  • Subscription and billing information (processed by Stripe — see below)

Information collected automatically:

  • Basic website analytics such as page views and general geographic region, collected through Cloudflare Web Analytics. This service is cookieless and does not track individual visitors across websites or sessions.

We do not use cookies for tracking, advertising, or personalization. We do not collect device identifiers, IP addresses tied to individual profiles, or behavioral data for advertising purposes.

How We Use Your Information

We use the information we collect for the following purposes:

  • To create and manage your WoundScore account
  • To process your subscription and send billing-related communications
  • To send you product updates, feature announcements, and support communications related to WoundScore (transactional email)
  • To send marketing emails about WoundScore, if you have opted in or if we have a legitimate interest based on your professional relationship with us
  • To respond to support requests and inquiries
  • To comply with applicable legal obligations

We do not sell your personal information. We do not share your personal information with third-party advertisers. We do not use your information for behavioral advertising or cross-context tracking.

Sub-Processors and Third Parties

We use a limited set of trusted third-party service providers to operate this website. Each is contractually required to protect your information and use it only to provide services on our behalf.

Clerk (Authentication)

Clerk handles account login and identity verification. WoundScore.com does not store passwords. Clerk stores authentication credentials under their own security controls. Learn more at clerk.com/privacy.

Stripe (Payment Processing)

Stripe processes all subscription payments. WoundScore.com never stores credit card numbers or payment card data. Stripe is PCI-DSS Level 1 certified. Learn more at stripe.com/privacy.

Resend (Transactional Email)

Resend delivers account confirmation, billing notifications, and other transactional messages to your email address. Learn more at resend.com/privacy.

HubSpot (Marketing Email and CRM)

HubSpot manages our marketing email list and customer relationship data. If you receive marketing communications from us, your name and email address are stored in HubSpot. You can opt out at any time using the unsubscribe link in any marketing email. Learn more at hubspot.com/legal/privacy-policy.

Cloudflare (Hosting and Analytics)

WoundScore.com is hosted on Cloudflare Pages, which is US-based infrastructure. Cloudflare Web Analytics collects aggregate, cookieless analytics. No individual user profiles are built. Learn more at cloudflare.com/privacypolicy.

Contentful (Content Management)

Contentful powers the content management layer for this website. It does not process personal information collected from visitors.

We do not use Google Analytics, Meta Pixel, or any other advertising tracking technologies on this website.

How Long We Keep Your Information

We retain your account information for as long as your account is active. If you cancel your subscription and close your account, we will delete your personal information within 90 days, except where we are required to retain it for legal or tax compliance purposes (typically up to seven years for billing records).

Marketing email list data is retained until you unsubscribe or request deletion.

Your Rights

Regardless of where you are located, you can:

  • Access your information: Request a copy of the personal information we hold about you.
  • Correct your information: Ask us to fix inaccurate or incomplete information.
  • Delete your information: Request that we delete your personal information. We will fulfill deletion requests within 45 days, subject to any legal retention requirements.
  • Opt out of marketing email: Use the unsubscribe link in any marketing email, or contact us at privacy@woundscore.com. We will process opt-out requests within 10 business days.

California residents have additional rights under the California Consumer Privacy Act (CCPA/CPRA), including:

  • The right to know what categories of personal information we collect and how we use them
  • The right to delete personal information we have collected from you
  • The right to correct inaccurate personal information
  • The right to opt out of the sale or sharing of personal information (we do not sell or share your personal information, so this right is already satisfied by our practices)
  • The right not to be discriminated against for exercising your privacy rights

To exercise any of these rights, contact us at privacy@woundscore.com or by mail at the address below. We will verify your identity before fulfilling any request and will respond within 45 days. If we need additional time, we will notify you within the initial 45-day period.

California residents may submit requests twice per year at no cost.

Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect your information against unauthorized access, disclosure, alteration, or destruction.

Authentication is handled by Clerk. Payments are handled by Stripe. Neither service causes raw credentials or card data to pass through our systems.

No internet-based system is completely secure. If you have concerns about the security of your account, contact us at privacy@woundscore.com.

Breach Notification

In the event of a security incident that affects your personal information, we will notify you as required by applicable law, including the FTC Health Breach Notification Rule where applicable. We will provide notification without unreasonable delay and no later than 60 days after discovering a breach, unless law enforcement requests a delay.

Because WoundScore.com does not handle PHI, HIPAA breach notification requirements do not apply to this website.

Children

WoundScore.com is a professional platform for licensed healthcare clinicians and agency administrators. We do not knowingly collect personal information from anyone under the age of 18. If you believe a minor has provided us with personal information, contact us at privacy@woundscore.com and we will delete it promptly.

Changes to This Policy

We will post any updates to this Policy on this page with a revised "Last Updated" date. If we make material changes, we will notify you by email (using the address on your account) at least 30 days before the changes take effect. Your continued use of WoundScore.com after the effective date of any changes constitutes acceptance of the updated Policy.
